Join Our Team as a Nurse Practitioner

Hueman Risk Adjustment Solutions is actively seeking a skilled Nurse Practitioner to join our team in providing in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in the Olmsted Falls, OH area.

About the Opportunity

This unique position offers a flexible schedule, allowing you to balance your professional goals with your personal lifestyle. As a Nurse Practitioner, you will have the opportunity to interact with patients in a more intimate setting, conducting comprehensive, non-invasive physical assessments and providing basic patient education on wellness and preventative healthcare measures.

Responsibilities
  • Travel to and from patients' homes to conduct HRAs
  • Perform comprehensive physical assessments and review medical history
  • Ensure accurate and timely documentation and conduct medical reconciliation to ensure patient safety and compliance
  • Provide patient education on wellness and preventative healthcare measures
Requirements
  • Master's Degree or commensurate experience and satisfactory completion of NP license
  • Active State NP license
  • Active National board certification
  • Current BLS certification
  • 12 months of experience as a paid advanced clinical provider preferred but not required
