Job Summary

We are seeking a highly skilled Speech Pathologist to join our Home Health team at Yale New Haven Health. As a Speech Pathologist, you will be responsible for providing high-quality patient care and rehabilitation services to patients in their homes.

Key Responsibilities
  • Clinical Services
    • Conduct comprehensive evaluations and assessments of patients' communication and swallowing disorders
    • Develop and implement individualized treatment plans to address patients' needs
    • Provide therapy sessions to patients in their homes, utilizing a variety of techniques and modalities
  • Communication and Collaboration
    • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
    • Collaborate with other healthcare professionals to develop and implement comprehensive care plans
  • Documentation and Reporting
    • Maintain accurate and timely documentation of patient progress and treatment plans
    • Prepare and submit reports to healthcare team members and payers as required
Requirements
  • Education
    • Master's degree in Speech-Language Pathology from an accredited program
  • Experience
    • At least one year of experience as a Speech Pathologist in a home health or hospital setting
  • Licensure
    • Current licensure as a Speech-Language Pathologist in the state of Connecticut
  • Skills
    • Excellent communication and interpersonal skills
    • Ability to work independently and as part of a team
    • Strong organizational and time management skills
Preferred Qualifications
  • Experience with FEES, MBS, and TEP
  • Experience working with children and/or adults
  • Experience with home health care
