About the Role:- Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Licensed in Minnesota as a Practical Nurse with no restrictions.
- Ability to communicate effectively, follow instructions, and take clear notes.
- Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification or ability to obtain certification within the probationary period.
- Experience working in a long-term care environment.
- Experience working with Veterans.
