Position Overview
ChristianaCare is actively seeking a dedicated full-time Registered Nurse for our Primary Care Services. This role is primarily based in our community health practice, where the nurse will also provide telephonic triage support for our additional primary care location.
Work Schedule
- Monday to Friday
- Daytime hours
- No weekend or holiday shifts
Key Responsibilities
- Exhibits the knowledge and skills necessary to deliver care tailored to pediatric, adolescent, adult, and geriatric patients, whether virtually or in-person.
- Engages in a relationship-centered approach to healthcare delivery.
- Conducts Nurse Visits, including Annual Wellness Assessments, Advance Care Planning, and Transitional Care Management.
- Administers point-of-care tests, EKGs, and other evaluations as instructed by healthcare providers.
- Triages walk-in patients, ensuring appropriate care and follow-up.
- Responds to patient inquiries and messages under the guidance of the healthcare provider.
- Assists healthcare providers during examinations and procedures.
- Acts as a communication bridge between patients, healthcare providers, and staff.
- Collaborates with team members to ensure a seamless and efficient patient flow.
- Addresses patient crisis situations effectively.
- Ensures adequate staffing coverage within the unit as necessary.
- Provides patient education and wellness counseling.
- Conducts medication history assessments.
- Supports the roles of LPNs and/or MAs in the practice.
- Contributes to the education and evaluation of clinical competencies for new and existing staff.
- Facilitates the orientation of new personnel to ensure proper training.
- Conducts chart audits or oversees designees in this task.
- Periodically observes clinical support staff to ensure quality care delivery.
- Acts as a liaison for accreditation and safety monitoring initiatives.
- Administers medications, monitors vital signs, and performs routine treatments as prescribed.
- Stays updated with policies and participates in quality improvement efforts.
- Engages in special projects as assigned.
- Adheres to safety protocols and reports any unsafe conditions promptly.
- Performs other related duties as required.
Education and Experience Requirements
- Graduation from a state-approved or National League of Nursing accredited School of Nursing.
- Minimum of two years of experience as a Registered Nurse, with preference for experience in a physician's office.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) considered with a commitment to complete a BSN within three years.
Special Requirements
- Current RN licensure in the State of Maryland.
- BLS certification is mandatory.
