We are seeking a skilled and compassionate Patient Engagement Specialist to join our team at Upward Health. As a key member of our outreach team, you will play a critical role in connecting with patients, understanding their needs, and empowering them to take control of their health.

• Develop and maintain strong relationships with patients through regular communication and outreach efforts
• Analyze patient data to identify areas of opportunity for improved health outcomes
• Collaborate with our clinical team to develop personalized care plans that address the unique needs of each patient
• Provide education and support to patients to help them navigate the healthcare system and make informed decisions about their care

• 3+ years of experience in a related field, such as sales, healthcare, or outreach
• Excellent communication and interpersonal skills, with the ability to build rapport with patients from diverse backgrounds
• Strong analytical and problem-solving skills, with the ability to interpret complex data and develop effective solutions
• Able to work independently and as part of a team, with a focus on delivering exceptional results

• $65,000 - $80,000 per year, depending on experience
