Job Title: Home Health RN

Supplemental Health Care is seeking a skilled Home Health RN to join our team in Farmington, Missouri.

Job Description:

We are looking for a compassionate and experienced Home Health RN to provide high-quality patient care in a home setting. As a Home Health RN with Supplemental Health Care, you will have the opportunity to work with a diverse range of patients and families, and make a meaningful difference in their lives.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
  • Current Missouri RN license.
  • Minimum of 1 year of recent home health nursing experience.
  • OASIS/Medicare Certified Visits experience.
  • Electronic Charting experience.
  • American Heart Association BLS.
  • Valid U.S. Driver's License.
