We are a compassionate healthcare organization dedicated to enriching the lives of our cherished seniors. Our multi-disciplinary primary care program addresses the physical, behavioral, social, and cultural needs of Medicare-eligible Hispanic seniors.
Job SummaryThe Mental Health Therapist will provide individual and family therapy services, coordinating with the clinical team to ensure comprehensive care. This role requires exceptional interpersonal skills, a sympathetic nature, and bilingual proficiency in Spanish and English.
Key Responsibilities- Provide behavioral health counseling services and conduct patient assessments to establish therapy treatment goals.
- Administer individual and group therapy, maintaining thorough and confidential records of observations.
- Evaluate progress and effectiveness of therapy interventions, providing patients with information regarding resources and therapeutic activities in the community.
- Work directly with patients, families, providers, and community providers to ensure active and collaborative care.
- Assess, diagnose, and treat clients/patients as consistent with mental health therapy licensures.
- Develop and maintain good working relationships with outside community health and social agencies, reaching out to the community to develop new resources for meeting patient needs.
- Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling.
- 2+ years of professional experience recommended.
- Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) licensure.
