Position Summary:

Eventus WholeHealth delivers comprehensive, evidence-based healthcare services to residents of skilled nursing and assisted living facilities through a multidisciplinary team of healthcare providers, including physicians, nurse practitioners, physician assistants, and support staff.


Essential Duties and Responsibilities:


Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for family medicine, geriatric medicine, and internal medicine as it pertains to long-term care facility work.

Review and sign MARs as requested by facilities, sign all orders specific for a supervising physician while at the facility or within 24 hours if faxed or emailed, including phone orders.

Manage scheduled medication prescriptions specific to each facility's patients and specialty, adhering to Eventus WholeHealth's scheduled medication RX policy.

Provide adequate time for paperwork and administrative duties during each visit.

Collaborate with facilities to manage systems, attend care plan meetings, and QA/PI meetings as requested by the facility or supervisor.

Provide on-call triage duties according to the on-call triage protocol policy.

Be available to facilities between 7 AM and 7 PM on scheduled workdays, with flexibility to adapt to changing triage schedules.

Submit documentation and billing records to Eventus WholeHealth according to established billing schedules.

Establish and maintain open, positive communication with facility staff and administration, providing verbal and written instruction or feedback regarding medications and other pertinent caregiver information.

Maintain patient confidentiality, adhering to HIPAA rules and regulations and CPT coding guidelines.

Be knowledgeable of and comply with all regulatory agencies' rules and guidelines, as well as professional ethical standards.


Qualifications/Education:

Licensed MD or DO

3 years of experience in Long Term Care/Geriatric Care

AMDA Certification a plus, but not mandatory

Strong leadership and organizational skills

Excellent written and verbal communication skills
