The ideal candidate will have strong communication skills, be able to work independently, and possess a strong commitment to patient-centered care.
Responsibilities:- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and providing vital sign assessments.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduation from an accredited School of Practical Nursing.
- Current licensure as a Licensed Practical Nurse in Pennsylvania.
- One or more years of experience in a home health agency or hospital setting, with home health experience preferred.
- Ability to pass a criminal background check and maintain current CPR certification.
