Medical Professional for Home Risk Assessments

The position involves conducting comprehensive health assessments in the comfort of patients' homes, reviewing their medical history, medication adherence, social environment, and other risks. This provides unparalleled insight into patients' overall health and well-being, allowing us to identify potential care gaps and improve quality of care.
We are seeking experienced Nurse Practitioners or Physician Assistants with strong communication skills and a passion for delivering exceptional patient care. As a member of our team, you will have the opportunity to work collaboratively with physicians, case managers, and other healthcare professionals to provide comprehensive care.
Responsibilities:
- Conduct adult and geriatric assessments, medical history reviews, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up support.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements:
- A Master's Degree or equivalent experience and licensure required.
- Current RN and NP or PA licensure in the state of practice with prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACN in a Matrix-approved specialty.
- A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- A positive and outgoing personality, and a team player with flexibility in frequent change.
Compensation and Benefits:
We offer competitive compensation ranging from $2,300 to $11,500 per month, based on completed visits and availability. Our benefits package includes a mileage reimbursement plan, 401(k), and limited voluntary benefits.
Location and Travel:
This role is based in Sidney, New York, with opportunities for travel within the state and potentially out of state. You must have a valid state driver's license and adequate automobile insurance coverage.
