Job Summary

The Care Coach Medical Assistant is a vital member of our healthcare team, responsible for providing high-quality patient care and education in a dynamic and fast-paced environment.

Key Responsibilities:

  • Conduct patient visits to promote established care plan goals and reinforce lifestyle modification education
  • Collaborate with the Care Educator Nurse to discuss cases and ensure patient care gaps are closed
  • Identify and address patient care gaps, providing education and support to promote health and wellness
  • Coach patient group workshops as organized by the Care Program Nurse Manager
  • Accurately document patient information into the EMR system
  • Participate in health fairs and health promotion events
  • Perform other assigned duties by the Care Program Nurse Manager

Requirements:

  • Nationally accredited Medical Assistant (MA) program with active MA national certification
  • Experience in Microsoft Word, Excel, and PowerPoint
  • Ability to think critically and solve problems in a fast-paced environment
  • Excellent communication and interpersonal skills
  • Ability to maintain accurate and detailed records
  • Certified Clinical Health Coach (CCHC) certification required within 4 months of employment
  • CPR/BLS for Healthcare Providers certification
