Job Summary: We are looking for a skilled and experienced Hospice Care Professional to provide high-quality patient care in a travel nursing setting.
Responsibilities:
- Provide direct patient care to patients in a hospice setting, including administering medications, wound care, and other treatments as needed.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and document findings in a timely and accurate manner.
- Develop and maintain strong relationships with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
- Participate in ongoing education and training to stay current on best practices and evidence-based care.
Requirements:
- Current RN licensure in the state of Massachusetts.
- Minimum of 1 year of experience in hospice care or a related field.
- Ability to work 32 hours per week, with flexibility to work varying shifts, including days and evenings.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
