Responsibilities:

Matrix Provider – What to Expect:
Conduct assessments for adults and geriatrics, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with Matrix protocols.

Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and relevant ancillary medical personnel as necessary.

Engage with Primary Care Physicians (PCP) on patient education and provide necessary follow-up.

Deliver services across various settings, including home visits, skilled nursing facility visits, and telehealth consultations.

Nurse Practitioner or Physician Assistant - PRN/Part-Time - Home Risk Assessments

Type:
PRN/Part-Time - Flexible-Nurse Practitioner

PAY:
$90.00 or $115.00 based on county per completed assessment.

Location:
Home, Skilled Nursing Facilities, Telehealth, and other community environments.

Hours:
Days, Evenings, Nights, Weekends, Flexible Scheduling.

Benefits:
PRN/Part-Time providers are non-benefited except for 401(k) and limited voluntary benefits.

Mileage reimbursement plan.

Qualifications:

Must Haves:
Master's Degree required OR equivalent experience and successful completion of NP licensure.

Master's Degree required OR equivalent experience and successful completion of PA licensure.

Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.

Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.

Current BLS, ACLS, or CPR certification.

A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant. Ability to travel, both in-state and out-of-state.

Strong computer skills and familiarity with EMR software.

Excellent verbal and written communication skills with patients, clients, and colleagues.

A positive and outgoing personality, with the ability to work collaboratively in a team environment. Comfort and flexibility with frequent changes.

Travel Requirements:
Ability to travel, possess a valid state driver’s license, and maintain proof of adequate automobile insurance coverage.

Travel required to meet patients in their residences.
