Role Responsibilities:
The Nursing Coordinator serves as the primary contact for patient transfers to the most suitable Avera facility. This role involves collaboration to assess each patient's individual needs and determining the best placement.
The Transfer Center offers various services, including:
• Arranging for accepting physicians
• Coordinating patient placements
• Ensuring open lines of communication
• Confirming bed availability.
This position is essential for tracking assignments and resolving issues to facilitate timely patient placements. The Nursing Coordinator collaborates with the Bed Placement Coordinator, nursing departments, external facilities, and physicians to ensure efficient patient transitions within the healthcare system. This role also involves gathering clinical information to maintain continuity of care and appropriate patient placement. The Nursing Coordinator monitors all transfers, identifies potential delays in care, and facilitates communication among care team members.
Qualifications:
- Graduation from a CCNE or NLN accredited nursing program is strongly preferred; graduates from an approved nursing program are acceptable.
- A Bachelor's degree in nursing is preferred.
- A minimum of two years of nursing experience is preferred.
- Previous experience in patient placement and logistics is preferred.
Licensure and Certification:
- Must be licensed as a Registered Nurse in South Dakota or meet multi-state licensure requirements.
- Specialty certification is preferred.
- Membership in a professional nursing organization is preferred.
Work Schedule:
- Hours include a rotation every third weekend and holiday.
Compensation:
The hourly wage for this position is competitive and based on experience.
