This full-time position involves providing rehabilitative care and skilled nursing to clients in their homes, allowing them to receive the medical care required to remain at home.

As a Physical Therapist with BAYADA, you will be an integral member of a multi-disciplinary health care team, working closely with clients, family members, and other healthcare professionals to develop and implement individualized care plans.

Responsibilities include:

  • Conducting home visits to clients in designated geographic territories.
  • Performing diagnostic tests and measurements, such as mobility and range of motion assessments.
  • Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Continually assessing and revising care plans, and participating in multidisciplinary care planning as needed.
  • Educating and instructing clients, family members, or other client representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.
  • Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided.

Qualifications for this position include:

  • A current VA Physical Therapist license.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
