Primary Care Physician Job Summary

We are seeking a skilled Primary Care Physician to join our team at the Windward Community Based Outpatient Clinic in Kaneohe, HI. As a Primary Care Physician, you will provide comprehensive outpatient care to a panel of adult patients using the Patient Aligned Care Team (PACT) model of care delivery.

Key Responsibilities:
  • Provide high-quality primary care to a panel of assigned patients, including routine check-ups, health screenings, and disease management.
  • Collaborate with other healthcare professionals to ensure seamless transitions in care and achieve optimal patient outcomes.
  • Contribute to the development and implementation of performance measures to improve access to care, quality of care, efficiency, and patient satisfaction.
  • Prescribe medications and enter consults to specialty clinics using electronic medical records systems.
  • Practice in accordance with professional standards, core values, and a commitment to excellent service.
  • Complete clinical reminders for chronic disease management and prevention when due.
  • Complete progress notes, encounters, secure messages, and view alter notifications according to VHA timeliness standards.
  • Provide and coordinate clinical-related Employee Health services working in conjunction with the Human Resources department.
  • Assist in completing Environmental Agents Registry exams.

Work Schedule: Monday through Friday, 7:30am to 4:00pm
