MCRT Practitioner

At Access, we are seeking a highly skilled MCRT Practitioner to join our dynamic team. The Mobile Crisis Response Team (MCRT) was designed to serve individuals of all ages who are experiencing a behavioral health crisis.

• Build relationships with clients to develop life goals and enhance engagement in recovery.
• Link clients to resources, services, and natural supports to support attainment of recovery goals and reduction of isolation.
• Work with clients to increase positive social connections and develop stronger family connections, improve life skills, and empower clients to increase wellness self-management.
• Document all interactions in the Electronic Medical Record (EMR).
• Promote linkage to services by assisting clients with appointment coordination, providing reminders, and escorting or driving to appointments as needed.
• Triage calls and assess an appropriate clinical response as authorized by the licensed clinician on the team.
• Participate in program staff/case conference meetings to promote efficient communication and care.
• Assist in assessing individuals to provide support needed to divert an ER visit or hospitalization.
• Develop exceptional follow-up diversion planning for individuals served by the Mobile team.

• Bachelor's degree in a Human Services field required.
• 1-3 years of experience in a Human Service-related field and/or lived experience as a consumer of behavioral health services or experience with Mental Health population.
• Demonstrated peer support or mental health experience.
