Family Medicine Physician

The University of California, San Francisco (UCSF) is seeking a highly skilled Family Medicine Physician to join our team. As a Family Medicine Physician, you will be responsible for providing primary medical care to patients of all ages, including acute and chronic problems, and preventative health needs. You will work in a team-based primary care delivery model, collaborating with consultants, ancillary care providers, and other members of the medical care team to optimize care coordination for patients in your primary care panel.

- Collaborate with consultants, ancillary care providers, and other members of the medical care team to optimize care coordination for patients in your primary care panel.
- Communicate and consult with the inpatient hospital team in the care of primary care patients who are admitted to the medical, surgical, or pediatric services.
- Provide on-call coverage for after-hours phone advice as a member of the pool.
- Complete medical records on all patient encounters in a timely fashion consistent with Primary Care Services policies.
- Eligible individuals should have completed an accredited family medicine residency program, be board certified or eligible in Family Medicine, and possess a valid California medical license to practice medicine in the State of California.
- Demonstrated excellent communication skills.
- Demonstrated ability to document encounters accurately and in a timely manner.
