Position Summary: The Case Manager at the Carolyn Williams Center (CWC) is tasked with embodying and promoting the mission of the Neighborhood Services Organization (NSO). This role emphasizes the importance of confidentiality and professionalism in all interactions and responsibilities.
The CWC Case Manager plays a crucial role in developing tailored case plans for young men participating in the program. The primary objective is to empower clients towards achieving personal and financial independence by addressing various obstacles to self-sufficiency. A key focus of this initiative is to assist clients in securing permanent housing and acquiring essential life skills to mitigate the risk of future homelessness.
The Case Manager fosters a supportive environment that prioritizes safety, structure, crisis intervention, advocacy, and collaborative problem-solving with clients. Building a professional rapport with clients in an empathetic and respectful manner is essential.
Key Responsibilities:
Case Management:
- Conduct initial assessments with clients to evaluate their physical and mental well-being and establish goals for their case plans.
- Communicate program rules, policies, and procedures to all new clients.
- Develop and implement individualized case plans aimed at promoting client self-sufficiency, including setting both short- and long-term objectives.
- Gather necessary documentation, such as employment records and medical reports, to support clients in accessing community resources.
- Hold regular progress meetings with clients to review their advancement towards goals related to education, employment, transportation, health, and finances, making adjustments to case plans as necessary.
- Maintain accurate and timely documentation of client interactions, including case notes and progress reports.
- Complete required statistics and reports, ensuring compliance with established deadlines.
- Consult with the Housing Director on program-related matters and individual client issues as needed.
- Encourage and facilitate client engagement in all program activities and housing requirements.
- Uphold client confidentiality and privacy in all interactions.
- Participate in the Housing On Call program as required.
- Ensure compliance with HIPAA regulations.
- Attend monthly housing meetings and represent NSO in various internal and external forums.
- Coordinate and oversee Life Skills training sessions for clients.
Property Management:
- Conduct regular inspections of the facility to ensure safety and maintenance standards are met.
- Report maintenance needs to the Facilities Manager promptly.
- Oversee and schedule client maintenance tasks to ensure the upkeep of the property.
- Perform room inspections as necessary and document findings in client files.
- Collaborate with clients and staff to maintain a clean and safe environment within the facility.
Additional Duties:
- Other responsibilities may be assigned by the Housing Director as needed.
- Process purchase orders for necessary supplies and food for the program.
Qualifications:
Minimum of an associate's degree in social work, human services, or a related field, along with two years of relevant experience. Additional experience may substitute for educational qualifications. A bachelor's or master's degree in a relevant discipline may be considered in lieu of specific field experience. Successful completion of a background check and drug screening is required.
A valid driver's license and auto insurance are necessary, along with reliable transportation for participation in the on-call schedule for after-hours housing issues.
Skills and Competencies:
- Strong oral and written communication skills are essential.
- Proficient in interpersonal communication, interviewing, and conflict resolution techniques.
- Ability to work independently and collaboratively within a team of volunteers, professionals, and clients.
- Patience and understanding when assisting the target population.
- Capability to manage multiple tasks and projects simultaneously.
Work Environment:
- Primarily office-based work in a well-lit, climate-controlled environment.
- Moderate physical activity is required, with occasional high-stress situations.
- Work schedule may vary based on the needs of the Center.
- Participation in the Housing Department's on-call rotation is required, which includes evening and weekend availability.
