- Collaborate with the interdisciplinary team to ensure seamless care transitions and minimize rehospitalizations.
- Delegate patient care responsibilities to staff and supervise them to ensure tasks and interventions are implemented effectively.
- Communicate patient information with assigned staff and between shifts to ensure continuity of care.
We are seeking a registered nurse with a strong background in care management and a passion for delivering exceptional patient care. The ideal candidate will have:
- A graduate degree in nursing from an accredited institution.
- Current licensure as a registered nurse in the state of New Mexico.
- One to three years of experience working as a RN in a related setting.
- CPR certification.
