About the Role

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Home Health Nurse. As a Home Health Nurse, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities
  • Deliver and Coordinate Quality Patient Care: Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders.
  • Assess and Document Patient Condition: Continuously observe and assess patient condition and care needs, and report changes in condition to the supervisor and/or physician as appropriate.
  • Implement and Update Care Plans: Participate in the development, implementation, and update of the nursing care plan to ensure patient needs are met.
  • Provide Education and Support: Educate patients and their families on disease management, self-care techniques, and prevention strategies to promote healthy outcomes.
  • Collaborate with Healthcare Team: Work collaboratively with the healthcare team, including physicians, therapists, and other healthcare professionals, to ensure comprehensive patient care.
Requirements
  • Graduate of an Accredited School of Nursing: Must be a graduate of an accredited school of nursing and hold a current, unrestricted state license as a Licensed Nurse in the state of practice.
  • CPR Certification: Must hold current CPR certification from the American Red Cross or American Heart Association.
  • Experience and Skills: Must have at least six months of recent experience as a Licensed Nurse in a clinical care setting, with a strong focus on home health care. Must possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
