Key Responsibilities
  • Conduct in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, to ensure the best possible care for our participants.
  • Collaborate with physicians and the care team to develop and implement individualized care plans that address the unique needs of each participant.
  • Work closely with the care team to coordinate care transitions, including hospitalizations and long-term care settings.
  • Provide regular visits to nursing home participants to ensure their ongoing care needs are met.
  • Participate in rotating night and weekend call to ensure 24/7 coverage for our participants.
Requirements
  • Graduation from an accredited school of nursing.
  • Certificate as a Nurse Practitioner with prescriptive authority.
  • RN state license.
  • Active DEA registration.
  • Eligibility to provide services to Medicare and Medi-Cal participants.
  • Minimum of two years of experience working with the senior population.
  • Current knowledge of chronic care and geriatric issues.
