Job Summary: We are seeking a skilled Triage Nurse RN/LPN to join our team at GraceMed Health Clinic. The successful candidate will be responsible for providing professional assessment of patient's health concerns over the telephone, without the advantage of visual inspection or face-to-face interaction.
Key Responsibilities:- Assessment and Triage: Utilize communication skills, knowledge of disease processes, and normal growth and development for all age groups to ascertain an accurate understanding of patient's symptoms.
- Severity Determination: Determine the severity of the patient's complaint; direct the caller to the appropriate emergency services if necessary; recommend suggested medical follow-up based on their assessments and established triage protocols.
- Referral Coordination: Coordinate referrals with physicians, specialists, and ancillary services on behalf of patients when needed.
- Patient Confidentiality: Maintain patient confidentiality at all times.
- Protocol Development: Assist in the development and documentation of protocols related to job functions.
- Education: Graduate from an accredited school of nursing required.
- Certifications/Licenses/Registrations: Current State of Kansas licensure as an RN required; current CPR certification (must be obtained within 30 days of employment).
- Experience: Experience in public health and/or critical care experience preferred.
- Office Software: Must possess a thorough knowledge of modern office practices, procedures, and equipment, including computers, copiers, and other standard office equipment.
- Problem-Solving: Ability to work independently, organize, monitor, and adjust work as necessary to ensure accuracy and timeliness.
- Communication: Demonstrated ability to identify and solve problems; ability to function independently and document meticulously.
- Team Player: Ability to strongly embrace and personify the mission and values of GraceMed with socio-economic and cultural sensitivity in mind.
- Communication Skills: Must display good verbal and written communication skills, and be able to professionally receive and follow oral instructions.
- Stress Management: Able to handle stressful situations.
The normal schedule is Monday through Friday 8:00 am to 5:00 pm, with the possibility of varying schedules. The work environment is a medical office setting, requiring the ability to sit and/or stand for long periods of time, with potential exposure to blood and body fluids and other hazardous substances.
