Job SummaryThe successful candidate will be responsible for providing professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations. This includes assessing patient needs, developing and implementing individualized care plans, and providing skilled nursing interventions to promote optimal health outcomes.
Responsibilities- Provide professional nursing services under the direction of a RN in compliance with state and federal regulations.
- Assess patient needs and develop individualized care plans to promote optimal health outcomes.
- Provide skilled nursing interventions, including wound care, medication management, and education.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in accordance with company policies and procedures.
- Graduate of an accredited Licensed Practical Nursing program.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
