Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for patients
- Collaborate with healthcare professionals to ensure seamless care coordination and communication
- Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
- Provide education and support to patients and their caregivers on disease management and wellness
- Participate in quality improvement initiatives to enhance patient outcomes and care delivery
- Completion of a Nurse Practitioner or Physician Assistant accredited program
- Current state RN license and NP/PA license
- Access to reliable transportation
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
- Knowledge of CPT, ICD-10, and HCPC codes
