Primary Duties:
- Perform comprehensive patient evaluations, addressing physical, emotional, and discharge needs.
- Develop and execute individualized care strategies, including medication management and clinical interventions.
- Document and relay patient status accurately to the healthcare team.
- Support patients with daily living activities and offer educational resources to patients and their families.
- Engage with interdisciplinary teams to guarantee holistic patient care.
- Champion the physical and emotional health of patients.
- Oversee and assign responsibilities to support staff.
Qualifications:
- Associate's Degree in Nursing is mandatory; Bachelor's Degree is advantageous.
- Strong analytical skills and the ability to function independently.
- Valid RN license and Basic Life Support certification are required.
- 1-3 years of experience in acute care settings is preferred.
