Resource Navigator

Position Summary:
The Navigator plays a crucial role in aiding individuals and families in accessing essential resources. This position encompasses conducting assessments, providing advocacy, facilitating referrals, coordinating services, and ensuring follow-up support.
Key Responsibilities:
- Gather pertinent information from clients and their families; evaluate and prioritize their needs; identify available resources; and establish rapport with clients.
- Coordinate a comprehensive network of services tailored to meet the unique needs and preferences of clients; serve as a liaison between clients and various agencies; assist clients in completing necessary applications for services.
- Develop and maintain relationships with service providers and resource networks.
- Keep accurate records as required.
- Engage in case consultations and participate in supervisory meetings.
Additional Responsibilities:
- Perform other duties as assigned.
Qualifications:
Experience and Skills:
- Minimum of one year of experience in a social service program is preferred.
- Experience in crisis intervention and management is preferred.
- Experience working with high-risk populations is preferred.
Skills Required:
- Exceptional verbal and written communication skills.
- Strong listening and customer service abilities.
- Proficient in basic mathematics and statistics.
- Competent in Microsoft Office and relevant software applications.
- Adept at reading, analyzing, and applying written procedures.
- Effective in researching and disseminating information.
- Ability to identify problems and propose solutions.
- Skilled in building trust and rapport with clients.
- Exhibit tact, discretion, and sound judgment.
- Experience working with diverse populations.
Education:
- High school diploma or equivalent is required.
- Pursuing further education in social work or a related field is preferred.
Other Requirements:
- Must pass background checks.
- Must possess a valid driver's license and reliable transportation.

Physical Requirements:
The physical demands of this position include frequent use of hands, sitting, talking, and hearing. Occasional standing, walking, reaching, climbing, and lifting up to 25 pounds may be required. Vision abilities include close and distance vision, color vision, and the ability to adjust focus.
Work Environment:
This role involves working in an office setting and in various residential environments. Employees may encounter unsanitary or unsafe living conditions and will be required to travel locally and long-distance, sometimes in adverse weather conditions.
