Job SummaryThe Case Manager RN will be responsible for assessing the health needs of our patients, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure timely and effective treatment. This is a full-time position, Monday-Friday, in Wilmington, DE.
Key Responsibilities- Assess patient health needs and develop comprehensive care plans
- Coordinate with healthcare providers to ensure timely and effective treatment
- Provide education and support to patients and their families
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Maintain accurate and up-to-date patient records
- Current RN licensure in the state of practice
- At least three years of experience as a Registered Nurse in an emergency room, occupational health clinic, or other ambulatory care clinic
- Occupational health experience required
- Knowledge of Internet software, Spreadsheet software, and Word Processing software
- Knowledge and experience with Electronic Medical Records preferred
