We are currently seeking a passionate and proactive PATIENT CARE NAVIGATOR to join our team. This role is essential in providing comprehensive care coordination for our patients, ensuring they receive the support they need throughout their recovery journey.
Key Responsibilities:
As a Patient Care Navigator, your primary duties will include:
- Developing individualized care plans and conducting thorough patient needs assessments.
- Facilitating life skills workshops in accordance with organizational policies.
- Establishing and maintaining collaborative relationships with hospitals, community health organizations, and state agencies to enhance patient services.
- Documenting patient admissions, care management requirements, and discharge plans.
- Administering assessments to identify patient needs and ensuring the Addiction Severity Index (ASI) is completed during intake.
- Coordinating patient schedules, managing transitions in care, and maintaining open communication with Medicaid Placement Coordinators.
- Assisting the treatment team with patient intake, transfer, and discharge evaluations.
- Supporting patients in accessing essential resources such as clothing, medical care, and legal assistance.
Qualifications:
To be considered for this position, candidates should possess:
- An associate's or bachelor's degree in human services or a related field is preferred.
- The ability to obtain a Certified Addiction Technician (CAT) certification within 18 months of employment.
- A solid understanding of substance use disorders, mental health issues, and treatment systems.
- Advanced computer proficiency.
- CPR and first-aid certification (initial training and renewals are covered by the agency).
- A valid driver's license and a clean driving record.
- Effective communication skills within a multidisciplinary treatment framework.
