Job Summary

As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes, while adhering to the highest standards of professionalism and confidentiality.

Key Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders, while maintaining accurate and timely documentation of all activities, assessments, and nursing actions.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Participate in the development and implementation of individualized nursing care plans, ensuring that patient needs are met and outcomes are achieved.
  • Provide education and support to patients and their families, promoting self-care techniques, prevention strategies, and disease management.
  • Maintain knowledge of competencies related to the nursing profession, participating in educational programs, continued education units, and skill evaluations.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical and Environmental Demands
  • Ability to speak, write, read, and understand English.
  • Ability to travel.
  • Ability to lift 50 pounds.
  • Ability to reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Ability to sit and climb stairs.
  • Visual and hearing acuity.
  • Strong sense of smell and touch.
Working Conditions
  • Ability to function in a wide variety of environments, including exposure to allergens and other conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
