As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes, while adhering to the highest standards of professionalism and confidentiality.
Key Responsibilities- Deliver and coordinate quality patient care in compliance with physician orders, while maintaining accurate and timely documentation of all activities, assessments, and nursing actions.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
- Participate in the development and implementation of individualized nursing care plans, ensuring that patient needs are met and outcomes are achieved.
- Provide education and support to patients and their families, promoting self-care techniques, prevention strategies, and disease management.
- Maintain knowledge of competencies related to the nursing profession, participating in educational programs, continued education units, and skill evaluations.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Ability to speak, write, read, and understand English.
- Ability to travel.
- Ability to lift 50 pounds.
- Ability to reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Ability to sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Ability to function in a wide variety of environments, including exposure to allergens and other conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
