Join Our Team as a Home Health Aide

We are seeking a compassionate and skilled Home Health Aide to join our team at SunCrest Home Health, a part of LHC Group. As a Home Health Aide, you will play a vital role in providing personalized care and support to our patients in the comfort of their own homes.

Responsibilities:
  • Provide patients with in-home personal care and designated health-related services to maintain their physical and emotional well-being.
  • Follow the written plan of care, Medicare/Medicaid regulations, and agency policies and procedures.
  • Perform incidental household services essential to the patient's health care at home.
  • Complete clinical notes for each visit within required timeframes.
Qualifications:
  • Successful completion of a competency evaluation.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
