Key Responsibilities:
- Provide education on Home Hemodialysis to patients and their caregivers
- Track and organize patient cases, ensuring timely and effective treatment
- Empower patients to take control of their health through dialysis administration and education
- Collaborate with support systems and care partners to ensure seamless care delivery

Requirements:
- Current driver's license and reliable transportation

- Previous experience in Chronic Hemodialysis, Acute Dialysis, Peritoneal Dialysis (PD), or Home Hemodialysis (HHD) nursing
- Training/Teaching/Healthcare Education background
- Previous case management experience
