Job Summary

The Behavioral Health Therapist I is a key member of the healthcare team, responsible for providing high-quality, patient-centered care to individuals receiving treatment in the hospital milieu setting. This role requires a strong understanding of psychosocial assessment, treatment planning, and discharge planning to ensure seamless continuity of care.

Key Responsibilities
  • Assessment and Treatment Planning
    • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized treatment plans.
    • Collaborate with the interdisciplinary team to develop and implement treatment goals and discharge plans.
  • Therapeutic Services
    • Provide individual, group, and family therapy sessions to patients and their support persons.
    • Facilitate counseling sessions to address a range of issues, including crisis intervention, problem-solving, and critical thinking.
  • Case Management and Discharge Planning
    • Coordinate discharge planning to ensure continuity of care and customer satisfaction.
    • Identify follow-up services, make appointments, and contact referral sources to ensure seamless communication of post-acute plans.
  • Communication and Collaboration
    • Serve as a primary liaison between community resources and linkage of patients and the Laurelwood program.
    • Provide treatment updates throughout patient hospital stay to all parties for seamless communication of post-acute plans.
Requirements
  • Licensure and Certifications
    • Licensure at Associate level required within six (6) months of hire.
    • Certified Addiction Counselor (CAC) II also acceptable.
  • Education and Experience
    • Master's degree in Social Work, Psychology, Counseling, or other related field that meets the requirements for licensure in the state of Georgia or meets the requirements for Certified Addiction Counselor (CAC II) accepted.
    • At least one (1) year interview and assessment experience with patients having psychiatric, emotional, or chemical dependency problems.
  • Skills and Abilities
    • Excellent interpersonal and communication skills as evidenced by the ability to work collaboratively with the healthcare team.
    • Knowledge of post-acute community resources and linkage for funding, housing, and multi-agency coordination of care.
    • Skilled at facilitating counseling sessions for group, individuals, and families.
