Role Summary
This position involves the comprehensive management of health care for residents, from pre-admission through post-discharge. The role focuses on the coordination and sequencing of care to enhance resident flow and foster interdisciplinary communication, which is vital for early intervention. The RN will evaluate outcomes based on established clinical, financial, and utilization data.
The responsibilities include interviewing potential residents and assessing their eligibility in accordance with regulations and policies. The RN will develop and continuously evaluate individualized service plans for each resident, ensuring compliance with assisted living qualifications.
Key Responsibilities
- Demonstrate independence and critical thinking in patient care.
- Maintain clear documentation and effective communication with residents and families.
- Coordinate care with other health care professionals.
- Monitor and evaluate patient responses using structured care methodologies.
- Manage resources responsibly and minimize variances.
Qualifications
A Bachelor’s degree in nursing is preferred, along with graduation from a nationally accredited nursing program. Candidates should possess a valid RN license and have at least one to two years of professional nursing experience in a clinical setting.
