Key Responsibilities:- Collect and document patient information to inform triage and office processes
- Provide medical guidance to patients with acute and chronic health conditions, as directed by providers
- Offer patient education on illnesses, diagnoses, symptom management, medications, and treatment plans
- Support care teams with telephone encounters and return calls to patients
- Transfer calls to appropriate staff or care teams
- Assist with medication prior authorizations and refills
- Administer nebulizers, medications, and vaccines according to the childhood immunization table
- Document encounters and decision-making in the electronic medical record (EMR) in a concise and accurate manner
- High School diploma or equivalent
- Completed an accredited Registered Nurse education program
- Active and unrestricted RN licensure in the state of MA
- Active CPR/BLS certification or willingness to obtain within 90 days of employment
- Ability to travel between multiple locations within 25 miles
- 1+ years of hands-on nursing experience
- EMR experience
- Bilingual English/Spanish and/or English/Portuguese
