Key Responsibilities- Collaborate with pharmacy leadership, physicians, and interdisciplinary team members to deliver pharmaceutical care in accordance with regulatory standards and hospital policies.
- Conduct patient and physician education activities as necessary to ensure optimal patient outcomes.
- Function as a source of drug information and provide expert guidance to healthcare professionals.
- Serve as a primary preceptor for pharmacy students and residents, providing mentorship and guidance in clinical practice.
- Implement decisions set forth by the Pharmacy and Therapeutics Committee and comply with hospital policies and procedures.
- A PharmD or Bachelor's of Science degree in Pharmacy is required.
- Completion of an American Society of Health-System Pharmacists PGY 1 residency or 3 years of applicable experience is required.
- Completion of an American Society of Health-System Pharmacists PGY2 residency in ambulatory care is preferred.
- Must be licensed to practice pharmacy in the state of Texas.
- Certification by the Board of Pharmaceutical Specialties in Pharmacotherapy or Ambulatory Care is preferred.
