Position Overview:

We are in search of a part-time, dedicated Nurse Practitioner to join our compassionate team. In this role, you will be instrumental in delivering holistic care to patients in long-term care settings.

This position offers a unique chance to positively impact the lives of patients while collaborating with a committed group of healthcare professionals.


Key Responsibilities:
  • Deliver comprehensive healthcare services and oversee patient management.
  • Collaborate closely with a multidisciplinary team of healthcare providers.
  • Evaluate, diagnose, and treat both the physical and mental health of residents.
  • Conduct detailed patient evaluations, including reviewing medical histories and performing physical assessments.
  • Order and analyze diagnostic tests.
  • Formulate precise diagnoses and treatment strategies.
  • Prescribe medications and administer necessary treatments.
  • Provide guidance and support to residents and their families.
  • Work with the interdisciplinary team to ensure seamless care coordination.
  • Manage chronic health conditions and track patient progress.
  • Adjust care plans as needed to optimize patient outcomes.
  • Commit to a person-centered approach to care.
  • Create a safe and supportive environment for elderly residents.

Qualifications:
  • Valid Nurse Practitioner License.
  • DEA License.
  • Minimum of 6 months experience in long-term care settings.

Compensation:

Part-time salary range: $90K - $120K annually.
