The Physical Therapist role at CHA PACE focuses on collaborating with the interdisciplinary team to support the therapeutic needs of program participants, educating and providing input on participant care plans, and maintaining a caseload of participants who can be treated in the home and in an outpatient gym setting located in our Adult Day Health Center.
Responsibilities- Evaluate and treat program participants in their homes and at the PACE Center Clinic, with a focus on quality care, service excellence, cultural responsiveness, and performance improvement.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the unique needs of each participant.
- Provide education and training to participants and their caregivers on physical therapy techniques and strategies to promote independence and mobility.
- Maintain accurate and timely documentation of participant progress and care plans.
- Participate in quality improvement initiatives to enhance the delivery of physical therapy services and improve participant outcomes.
- Bachelors, Masters, or Doctoral Degree in Physical Therapy.
- Massachusetts PT License.
- Maintains current American Heart Association or American Red Cross BLS for Healthcare Providers certification.
- At least 1 year PT experience preferred.
CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement.
CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
