Job Summary

Acentra Health is seeking a skilled Clinical Assessor to join our team. As a Clinical Assessor, you will play a critical role in determining the eligibility of North Carolina Medicaid beneficiaries for personal care services and home and community-based waivers.

Key Responsibilities
  • Conduct thorough assessments to determine the level of care required by beneficiaries.
  • Ensure that personal care services are provided on a needs basis, in quantities appropriate to the beneficiary's unmet needs.
  • Develop and implement service plans that meet the unique needs of each beneficiary.
  • Collaborate with healthcare professionals, family members, and caregivers to ensure that beneficiaries receive the necessary support and services.
  • Maintain accurate and up-to-date records of assessments, service plans, and beneficiary interactions.
  • Participate in training and professional development opportunities to stay current on best practices and regulatory requirements.
Requirements
  • Registered Nurse or LCSW license by the state of North Carolina.
  • 2+ years of experience in home care or a related field.
  • Strong assessment and evaluation skills, with the ability to develop and implement effective service plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality services.
