Job Responsibilities:- Assess the patient's physiologic health status and develop a plan of care to ensure optimal outcomes.
- Conduct patient interviews, explain policies and procedures, and review patient charts to ensure accurate and timely care.
- Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status, and adjust the plan of care as needed.
- Communicate and document patient information to ensure seamless care coordination.
- Provide emotional support to patients and their families, and educate them on health and wellness topics.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Perform various procedures, including decubitus care, skin care, and medication administration, with precision and attention to detail.
- Participate in patient and family education, and provide discharge instructions to ensure a smooth transition to home care.
- Graduate from an accredited School of Nursing, with a Bachelor's Degree in Nursing preferred.
- Current license to practice as a Registered Professional Nurse in New York State.
- BCLS and ACLS certifications preferred.
