Responsibilities:

* Conduct complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
* Collaborate with physicians, office staff, and ancillary departments to ensure patient education and support post-acute needs.
* Develop and implement transition plans, including referrals to local, state, and federal agencies as needed.
* Lead and facilitate multi-disciplinary patient care conferences, including Complex Case Review.
* Communicate effectively with patients, families, and caregivers to ensure understanding and support of the plan of care.

Requirements:

* Master's of Social Work (MSW) degree required.
* 2 years of acute hospital experience preferred.
* Current licensure, certification, or registration to practice as a social worker in the state of practice or policy.
* Accredited Case Manager (ACM) certification preferred.

Physical Demands:

* Lift/position up to 25 lbs.
* Push/pull up to 25 lbs of force.
* Frequent sitting.
* Moderate standing, walking, reaching, stooping, and bending.
