Physical Therapist

The Physical Therapist is responsible for providing high-quality patient care in a dynamic and supportive environment. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
Provide assessment, treatment, and intervention to patients with performance dysfunction or potential for dysfunction
Evaluate treatment outcomes to assess effectiveness
Coordinate the transition of patients to facilitate or prepare for change from one functional level to another
Document plan of care and all services provided
Requirements:
Master's Degree in Physical Therapy
Current Physical Therapist Certification in the state of Operations
3+ years experience in Home Health, Acute Care setting or Rehabilitation Facility
