We are seeking a skilled PRN Social Worker to join our team at Arroyo Grande Community Hospital. As a PRN Social Worker, you will play a vital role in ensuring the medical, psychological, and social needs of our patients are met.
Key Responsibilities- Work closely with patients, caregivers, and the healthcare team to ensure seamless delivery of care.
- Interface with insurance carriers to facilitate continued coverage and authorization for next level of care.
- Assist the Program Director in assessing internal patients for admission to the Rehab Unit.
- Develop and implement case management, social work, and discharge planning services for the Acute Rehabilitation Unit.
- Maintain up-to-date knowledge of changes in community resources and Medicare regulations.
- Stay informed of CARF standards and Medicare regulations for inpatient acute rehabilitation.
- Master's Degree in Social Work from an accredited college or university.
- Current Social Work license (MSW) or eligibility, if required by state practice laws.
- Experience in the provision of Social Service in acute rehabilitation, preferably in a healthcare setting.
- Strong leadership skills and ability to work with individuals, families, and groups.
- Knowledge of basic services provided by various health and welfare organizations in the community.
- Current CPR certification, if required by the facility assigned to.
- Computer skills and knowledge of basic computer programs.
