Overview:
The Licensed Practical Nurse (LPN) serving as a Medication Care Manager plays a pivotal role in delivering exceptional care and services by administering medications and treatments in a structured and secure manner.

Medication Administration:
Deliver the highest standard of care by administering medications and treatments efficiently. Responsibilities include:
  • Documenting and reviewing the Daily Log to stay informed about residents' needs.
  • Administering medications while adhering to the Six Rights of Medication Pass.
  • Maintaining confidentiality regarding resident information.
  • Restocking and organizing the medication cart.
  • Monitoring and assessing residents for necessary medications.

Maintain cleanliness and organization of the Medication Room and treatment carts, ensuring compliance with safety standards.


Resident Care:
Engage with residents and families, addressing concerns through established processes. Responsibilities include:
  • Participating in daily meetings and health updates.
  • Contributing to the development of Individualized Service Plans.

Risk Management and Safety:
Collaborate with the community team to ensure compliance with safety regulations. Key tasks include:
  • Practicing safety procedures and reporting any incidents immediately.
  • Ensuring all equipment and hazards are managed according to safety codes.

Team Collaboration and Training:
Actively participate in team efforts and commit to achieving collective goals. Responsibilities include:
  • Engaging in regular training and meetings.
  • Contributing to team member engagement initiatives.

Qualifications:
To excel in this role, candidates must possess:
  • Valid LPN or LVN credentials.
  • CPR and First Aid certifications.
  • Experience working with seniors is preferred.
  • Strong communication skills and the ability to handle multiple priorities.
