Responsibilities:
* Assess patient/family response to care and report changes to the RN case manager
* Apply preventative, therapeutic, and rehabilitative nursing procedures and techniques
* Assist RN and/or physician in performing specialized duties related to end-of-life care
* Prepare clinical and progress notes timely and accurately
Qualifications:
* Graduate of an accredited school of practical nursing
* Current LPN licensure
* Minimum of 6 months of general nursing experience preferred
* Excellent nursing judgment and documentation skills
* Excellent oral and written communications and interpersonal skills
* Current CPR certification preferred
