Nurse Practitioner

We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.
Responsibilities:
Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans
Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management and self-care
Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure seamless care coordination and communication
Educate patients and their families on disease management, self-care, and health promotion
Requirements:
Current RN licensure in the state of practice
Current CPR certification
At least one year of clinical experience as a Registered Nurse, with a focus on home health or gerontology
