About the Role

Care Management is a collaborative practice model that brings together patients, nurses, social workers, physicians, other practitioners, caregivers, and the community to provide excellent care. As a Care Manager RN, you will facilitate care along a continuum through effective resource coordination, ensuring that patients receive the right care at the right time.

Key Responsibilities

  • Develop and implement individualized care plans that address the unique needs of patients
  • Coordinate with healthcare teams to ensure seamless transitions of care
  • Provide education and support to patients and their families to promote self-management and empowerment
  • Collaborate with community resources to address social determinants of health
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
