At Cross Country Nurses, we are seeking a skilled Travel Nurse RN Case Manager to join our team. As a case management nurse, you will play a vital role in collaborating with the healthcare team to assess, plan, facilitate and coordinate care for individual patients.

Job Description

Overview: We are looking for an experienced Registered Nurse with a background in case management to work on a travel nursing assignment in Clackamas, Oregon. This is a 13-week contract position with 40 hours per week, requiring 8 hours of work per shift.

Responsibilities:

  • Collaborate with the healthcare team to develop and implement patient care plans
  • Foster effective communication between patients, families, healthcare professionals, and social workers
  • C Coordinate services and resources to promote patient safety, quality of care, and cost-effective outcomes

Requirements:

  • At least 1 year of recent acute care experience in specialty
  • Active RN license
