Key Responsibilities- Conduct thorough social services evaluations to identify residents' needs and develop personalized care plans.
- Collaborate with residents, families, and healthcare teams to ensure seamless transitions and optimal care outcomes.
- Provide or arrange counseling services, psychiatric medication management, and other necessary support to promote residents' physical, emotional, and social well-being.
- Ensure accurate and timely documentation in MDS care plans.
- Current social work licensure in the state.
- Bachelor's degree in social work preferred.
- One year of supervised social work experience in a healthcare setting is preferred.
