We are seeking a skilled Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for planning, organizing, and directing hospice care for patients and their families.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, treatments, and performing various medical procedures.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between care settings.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Graduate of an accredited school of nursing with a current license to practice in the state.
- One to two years of recent acute care experience in an institutional setting, with a preference for experience in home care.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
